Information we collect

When you submit the New Client Form, the details you entered are sent to us and to our intake system so a counselor can be assigned. You also receive a copy of the referral on the confirmation page to keep, email, print or fax. Any files you attach are uploaded to our website and stored there so a counselor can open them.

Completing the first step alone sends us your name and company, so that we can follow up if the referral is not finished.

How we use information

Information you send us through this form, or by phone, email, fax or mail, is used solely to evaluate and administer the vocational rehabilitation referral it relates to, and is shared only with the parties involved in that claim — the insurer or third-party administrator, the employer, treating providers, training providers and, where applicable, hearing and appeals officers.

Medical information

Medical and vocational records we receive are handled under the authorization signed by the injured worker and are retained in the claim file. Authorizations are valid for one year from the date of signature and may be revoked in writing at any time.
